Example: One of our players was playing a female elf priestess. We had just killed two minor demons, so we examined the treasure chest in the room to see if there was any good stuff in it. She goes over, opens it, sees a magical necklace and puts it on. It was a necklace of strangulation. The player had to make a new character, and we laughed for about 10 minutes before we could play on.

The first cave crawl my friends did involved a vampire and a warrior, travelling through a cave. The GM got the vampire to face wooden elementals. Big wooden elementals. Which, and this was the beautiful part, fired wooden stakes out of their arms...
Needless to say the vampire didn't last long

I've never deliberately plotted to kill off players, but will take advantage of assumptions to the fullest (such as by putting a stairway right behind a door - cures the group of breaking down doors).
And, oh, the gaming stories:
In regards to 'piss off the DM' as a style of play, two of my players chose to do that when we had some newcomers to the group. Consequently, I spent more time handling their antics (which included hauling a dungeon door around, as well as setting fire to the forest). By the time one of them decided to pick one of the newcomer's pockets, I'd had my fill. They failed their roll, the other characters noticed, and chaos ensued, leaving the irritants in a bloody heap on the floor. Those players complained and were asked to leave the group.
Then there was a GURPS/Cyberpunk session where the characters are infiltrating a building - there's a large open space before the stairs, and we said to one character, "Kris, go for the stairs, we'll cover you!" He did, and the GM rolled a critical hit. He then rolled location, and got a head shot. Kris' character didn't even make it halfway.

Apparantly he did a google search for "Calistoga D&D", and this topic came up. He explained that he was a DM here in little Calistoga (pop. 6000), and had found a coupla other guys who were interested in playing.
I called him, and he explained what his plans were and stuff, and we made a date for last saturday.
To make a long story short, it was ######ign sick. W ended up playing for 12 hours, and I hate a great time. All the guys were cool, not complete social outcasts as I had expected, and it went smoothly. We're playing again this coming saturday.
